Case Managers Network

Purpose:
The Case Manager's network is a group that meets bimonthly. At these meetings, case managers share information, collaborate and give presentations on their programs. This is a chance to network and find out about the different resources that are available for clients in North County. All Case Managers are welcome!
Outcomes/Accomplishments:

Created a resource and communication network with over 187 case managers participating

Holds 6 meetings a year, with 40+ case managers in attendance

Provides 12-14 unique programs presented to the case managers group annually

Presented the Expand Your Tool Chest Workshop Series which provided 6 unique workshops for our members.This workshop series was generously sponsored by the Nordson Foundation.
Goals:

Continue to bring professional trainings to our North County case managers which will give them the tools they need to give clients the best care possible.
